This is a record of what your supplier told you, not a certificate. Nothing here makes a product compliant, and nothing here is issued by IFRA. Keep the original documents; a safety assessor will ask for them, not for this.

Percent of each allergen in the fragrance compound, as your supplier declared it. The calculator works out what that comes to in a finished bar.

I have the supplier’s allergen declaration

Tick this even when nothing is above the threshold. “None to declare” and “nobody has looked” are different answers, and only one of them is a label check that happened.

In soap
